Shin Nippon Biomedical Laboratories Ltd. (hereinafter referred to as "this company") recognizes the importance of personal information protection in our advanced information society and strives to protect personal information based on the following policy.

We acquire personal information by lawful and fair means.

We use personal information within the range of intended use indicated at the time of acquisition, to the extent necessary in the conducting of business.

When sharing personal information with a third party or entrusting the handling of personal information to a third party, we conduct a strict assessment of the third party, and we provide the appropriate guidance and supervision in order to preserve necessary confidentiality.

We do not provide personal information to third parties without obtaining prior consent from the relevant party, except as stipulated by laws and ordinances.

1. We maintain the accuracy of personal information.
2. In order to prevent the loss, destruction, falsification, or leakage of personal information, we take appropriate information security measures against unauthorized access, computer viruses, etc.
3. We do not disclose personal information, such as by transmission, to outside parties.

We affirm that each individual has the right to request disclosure, correction, usage suspension, deletion, etc. of personal information, and if there is such a request, we respond promptly without objection.

1. We have appointed a personal information protection manager who carries out the appropriate management of personal information.
2. This company conducts training on the protection and proper management of personal information for officers, employees (including full-time employees, contract employees, temporary employees, part-time employees, temporary workers, etc.), and other related persons. We routinely handle personal information in an appropriate manner.

We comply with applicable laws and other standards regarding personal information.

In order to implement this policy, we have established the Personal Information Protection Compliance Program managed by our officers, employees (regular employees, contract employees, temporary employees, part-time employees, temporary staff, etc.), and other related personnel, in order to distribute information, and maintain and continually improve said program.

Please send us a typed request via post containing the purposes for the use of personal information withheld by this company and include that for notification, disclosure, correction, addition or deletion, suspension, or termination (hereafter, collectively referred to as “disclosure”) of the personal information. Please note that we cannot accept requests from customers by phone, fax, e-mail, etc.

In requesting disclosure, etc. of personal data, please attach the documents described in (1) or (2) below along with the request form.

1. For requests of personal information by the individual (identity confirmation documentation), one of the following is necessary:
   ① Copy of driver's license
   ② Copy of passport
   ③ Copy of health insurance card
   ④ Original copy of resident certificate
2. For requests of personal information by a representative (proxy confirmation), all of the following are necessary:
   ① Identity confirmation for individual in question (any one of the four items listed in (1) above)
   ② Proof of identity for the representative (any one of the four items listed in (1) above)
   ③ Confirmation documentation of proxy authority
   
   • For legal representatives: documentation confirming the right of legal representation (one copy of the family register) is necessary
   • For any other arbitrary representative: the personal name seal registration certificate for the individual in question and a letter of proxy stamped with the registered seal 

※ We discard proof of identity documentation sent within six months of receipt.

We reply in writing in response to your request for disclosure of personal data, etc., so please enclose a stamp for 822 yen within the envelope (82 yen for the type 1 envelope, 430 yen for registered mail, and 310 yen for delivery confirmation) with the request form.

※ If the amount is insufficient or not enclosed, we will contact you. If payment is not made within the prescribed period, it is assumed that there is no request for information, etc.
